A few things to note prior to booking:

  1. In the state of Minnesota, a Physical Therapist can see a patient for 90 days without a doctor’s order. If you have an order for Physical Therapy, we will not be limited to this 90 day timeline.

  2. Superior Physical Therapy is a fee-for-service clinic, so payment is due in full at each session. You may send self-claims to insurance for reimbursement if desired.

  3. Superior Physical Therapy is not a Medicare Provider, so law prevents us from providing Medicare patients with what would be considered “normally covered services.” If you are a Medicare Beneficiary and would still like to request treatment at our clinic, please call 651-212-5223.
